FMEA (Failure Mode and Effects Analysis) is a systematic method for identifying, analyzing, prioritizing and documenting potential failure modes, their effects on a system, product and process performance, and the possible causes of failure. Take a minute to review this brief five-slide guide to FMEA.
Why do we use FMEA’s? We use FMEA’s to:
- Identify potential failure modes and rate the severity of their effects.
- Identify critical process limits or parameters.
- Rank order potential process deficiencies.
- Help focus on prevention of process problems.
- Document and track actions taken to reduce risk associated with failure modes.

Download this FMEA template that you will use for this assignment.
You will continue your work as a quality analyst and Six Sigma project leader. Use the same topic for your Project Charter from Units 1, 2, and 3.
Using the template, develop a FMEA. Your FMEA should include the following:
- At least 2 Processes or Parts
- 3 failure modes for each part/process
- Failure effects, causes, detection processes, actions, and plan
- Include your severity, occurrence, and detection assessment
