unit 4 project 4

FMEA (Failure Mode and Effects Analysis) is a systematic method for identifying, analyzing, prioritizing and documenting potential failure modes, their effects on a system, product and process performance, and the possible causes of failure. Take a minute to review this brief five-slide guide to FMEA.

Why do we use FMEA’s? We use FMEA’s to:

  1. Identify potential failure modes and rate the severity of their effects.
  2. Identify critical process limits or parameters.
  3. Rank order potential process deficiencies.
  4. Help focus on prevention of process problems.
  5. Document and track actions taken to reduce risk associated with failure modes.

FMEA is an important tool for the Improve phase in DMAIC. Before you begin this assignment, this 4-minute video (Links to an external site.)Links to an external site. gives a quick FMEA overview.

Instructions:

Download this FMEA template that you will use for this assignment.

You will continue your work as a quality analyst and Six Sigma project leader. Use the same topic for your Project Charter from Units 1, 2, and 3.

Using the template, develop a FMEA. Your FMEA should include the following:

  • At least 2 Processes or Parts
  • 3 failure modes for each part/process
  • Failure effects, causes, detection processes, actions, and plan
  • Include your severity, occurrence, and detection assessment
